OS selector, Linux adds second listing in menu

Linux after first use will add a second version of Linux to the OS boot menu label it "Linux 2" and make it the default OS. If I boot into Vista, delete the "linux2" entry and make sure Vista is he default system with the next boot of Linux it will do it again. Also If I try to use a mouse in the boot menu it will crash so I have to tic the "No mouse" button. Why does this happen and what can I do to fix it?
