Incremental Backup Errors Out Every Few Days

It appears that it may be related to the "Create a Full Version Every X Incremental Versions" feature as I have it set to create a full version every 3 incremental, but when I check the drive there are 4 backups in that string and then it starts erroring out before starting the nightly backup.
When I try to start it manually, it says The Specified file does not exist: backups\my partitions_full_b2_s1_v1.tib which would be the right naming convention for the next full version, but it looks like the full version is never created.
I have notifications set to email me, but I don't get emails when it errors out.
