Your question is a little ambiguous. If you are asking about difference in functionality between True Image run under Windows and True Image run from any bootable media (ISOLINUX, Acronis loader Linux, BartPE, or WinPE) in general (with the difference between base True Image vs. True Image Plus Pak being Universal Restore and dynamic disk support), a few of the key differences are as follows:
-from bootable media, mounting images is not supported
-from bootable media, pre- and post-processing commands for tasks is not supported
-from bootable media, none of the file cleanup procedures are available
Running True Image from bootable media is used primarily for disk imaging (backup) and restore and file backup and restore. Adding a new disk is supported, as is disk cloning.

Ok but in my download area, There are two bootable ISO's On for the Pluspak and one for Trueimage there both the same size. Just wondering what the difference are between those 2.

One has the Plus Pack installed, the other doesn't. The Plus Pack add Universal Restore capability which is missing in the base True Image. Otherwise, they are the same (they even have the same file size, but they ARE different files).
Make bootable media from them both and see the difference yourself. It will show in the Restore method screen of the Recovery wizard. With Plus Pack there will be a check box for Universal Restore, without Plus Pack there will not be a check box.
